ОÑтрый калькулезный холециÑтит: рекомендации Ð’Ñемирного общеÑтва неотложной хирургии WSES от 2016 года
ОÑтрый калькулезный холециÑтит ÑвлÑетÑÑ Ð¾Ñ‡ÐµÐ½ÑŒ раÑпроÑтраненным заболеванием, некоторые аÑпекты которого оÑтаютÑÑ Ð½ÐµÐ²Ñ‹ÑÑненными. Ð”Ð»Ñ Ñ‚Ð¾Ð³Ð¾ чтобы уÑтранить Ñти белые пÑтна, Ð’Ñемирное общеÑтво неотложной хирургии (WSES) разработало подробные рекомендации. ОбÑуждены диагноÑтичеÑкие критерии, Ð°Ð½Ñ‚Ð¸Ð±Ð°ÐºÑ‚ÐµÑ€Ð¸Ð°Ð»ÑŒÐ½Ð°Ñ Ñ‚ÐµÑ€Ð°Ð¿Ð¸Ñ, выÑвление ÑопутÑтвующих камней общего желчного протока, Ð¸Ð´ÐµÐ½Ñ‚Ð¸Ñ„Ð¸ÐºÐ°Ñ†Ð¸Ñ Ð¿Ð°Ñ†Ð¸ÐµÐ½Ñ‚Ð¾Ð² выÑокого операционного риÑка, выбор времени операции, вид хирургичеÑкого вмешательÑтва и альтернативы хирургичеÑкому поÑобию. Более того, предложен алгоритм: как только диагноз уÑтановлен, поÑле оценки риÑка холедохолитиаза Ñледует предлагать выполнение лапароÑкопичеÑкой холециÑÑ‚Ñктомии вÑем пациентам за иÑключением имеющих выÑокий риÑк оÑложнений и Ñмерти. Данные рекомендации Ñледует раÑÑматривать в качеÑтве дополнительного инÑтрумента Ð´Ð»Ñ Ð¿Ñ€Ð¸Ð½ÑÑ‚Ð¸Ñ Ñ€ÐµÑˆÐµÐ½Ð¸Ñ, и они не заменÑÑŽÑ‚ клиничеÑкой оценки Ð´Ð»Ñ ÐºÐ°Ð¶Ð´Ð¾Ð³Ð¾ отдельного пациента.
